Entry Fee: $200.00 (includes GST)

Entry Rules (Please Read Carefully)

• Entries acceptable only from people working directly in petroleum exploration, production, service and supply.
• Local golfers must be a Medicine Hat Oilmen's Association member by July 31, 2009.
• Last years golfers are given first preference.
• 160 entries accepted. New entries on a first come, first in basis. Entry fee must be included.
• Last years "No shows" come under new entries. These include those not showing up for the Best Ball.
• Entry form must be completed in full and correctly, or it will not be accepted.
• Championship flight golfers must have a certified handicap. Please bring your card with you.
• Closing date for entries is August 1 , 2009 (must be received - not post marked).
• Entry fee includes a golf cart and barbecue tickets for the golfer and escort.
• Free golfing and lunch for escorts on August 22, 2009, at the Cotton Wood Coulee Golf Course.
• No refunds on cancellation after August 12, 2009.
• No post dated cheques will be accepted. NSF cheques void entry.
• Extra barbecue tickets can be purchased for $20.00 per person.
